A page with a citation-needed tag on every second sentence is not. The audit is priced separately and stands on its own: if it concludes the article is in reasonable shape, you have a written second opinion and no obligation to commission anything further.

Only where it is inaccurate, unsourced, or given weight out of proportion to its coverage. Properly sourced criticism belongs in the article, and trying to delete it usually draws more editor attention to the page than leaving it alone would.
By fixing what the tag describes. A citation tag needs sources added, a tone tag needs promotional language rewritten, a notability tag needs independent coverage demonstrated. Once the cause is genuinely resolved, the tag can be removed with an explanation on the talk page.
Usually because they read as promotional, carry no citations, or come from an account with an undisclosed connection to the subject. Transparent editing with a declared conflict of interest and solid sourcing behind each change is far more durable.
It depends on the change. Uncontroversial factual corrections can often be made directly with disclosure. For contested or substantial edits, proposing them on the talk page and letting independent editors implement them is both guideline-preferred and more durable.
